Bridge to Recovery Housing Program Policies and Procedures

Policies

  • A Recovery House is limited to a Maximum of two (2) Scholarship Recipients for the Grant Period
    • (each unique street address). Exceptions to this policy occur in only rare circumstances where an incoming resident is at immediate risk of homelessness and there is documentation showing other funding sources have been exhausted and are unavailable. Scholarship Recipients may be renewed for up to three (3) months of funding
  • We Do Not Provide Scholarships for Established Residents
    • Applications for financial assistance for a resident already living in the recovery house will not be approved.
    • This funding supports transition to recovery housing only. New residents should apply two weeks BEFORE ENTRY.
  • Allow 2 Weeks for Application Review.
    • Processing takes a minimum of 1 week.  Requests for “expedited” review should ONLY be made in case of Emergency if Applicant is: 1) at immediate risk of homelessness AND 2) not transitioning from residential treatment.  
  • Other Funding Sources must be explored and unavailable to the Applicant
    • Treatment counselors and other referral sources should seek County and other public funding before applying for a Bridge Scholarship.

Recovery Housing Provider Forms Submission

  • Monthly (at end of each Month for which a Scholarship applies): 
    • Monthly Invoice uploaded via our Invoice Submission and End of Month Reporting Form on our Web Portal
  • One Time for each Resident at Discharge or End of Scholarship Month #3: (when a housing Provider will not be seeking further Bridge funding or renewal on behalf of the resident)
    • Outcomes Survey: Scholarship Recovery Capital Reporting Form

Form Submission Requirements and Policies:

  • IF AN INVOICE IS NOT RECEIVED ON OUR WEB PORTAL WITHIN 60 DAYS FROM THE RESIDENT’S DATE OF ENTRY AT THE RECOVERY HOUSE, THE FUNDS WILL BE FORFEITED AND WILL BE DIRECTED TO ANOTHER SCHOLARSHIP RECIPIENT.
  • All scholarship invoices (along with renewal requests) should be submitted by the end of the month for which the scholarship applies (the month being invoiced) on the Invoice Submission Form. 
  • If a resident is discharged from your Program, please remember to submit an Outcomes Survey which puts us on notice that further funding/renewals will not be needed and allows us to redirect those funds immediately to other residents in need.
